Laptop Class Request Form

Instructors must complete the Proposal Form for Laptop Use before requesting laptops for classes.

Laptop distribution subject to the number available.

I have completed the "Proposal Form for Laptop Use."

I have received approval from the VP of Academic Affairs for the Proposal Form for Laptop Use.

I have told my students they must have a Chattanooga State ID.

I have told my students they must return laptops and all parts at the time of check in.
